Community Wellness Announces Partnership with Kentucky Hospital Association

Through this new partnership, KHA and Community Wellness will work together to identify providers that can benefit from their turnkey, value-based care and reimbursement program.  By partnering with Community Wellness, Kentucky providers will have the opportunity to transition care into the home and further improve care coordination by monitoring patients’ vitals in real-time on a daily basis.  When a patient’s reading falls outside a “safe” parameter, they are notified, along with their physician, so steps can be taken to avoid a negative event – such as a hospital readmission.
