Creatinine is a catabolic product of creatine phosphate in muscle and protein metabolism. Creatine is synthesized primarily in the liver by methylation of S-adenosylmethionine to glycylamine and is then transported through the bloodstream to other organs, muscles and the brain. The conversion of creatine to creatine phosphate is catalyzed by creatine kinase, and creatinine is formed spontaneously during the reaction. Changes in creatinine levels may be an indicator of renal insufficiency or other medical conditions that cause decreased renal blood flow, such as diabetes or cardiovascular disease. Measurement of serum and urine creatinine levels is an important clinical test for kidney disease and dysfunction. Creatinine assay kits measure creatinine levels in samples.
In the creatinine assay, creatinine is converted to creatine by the enzyme creatinase, which is converted to creatine, which is specifically oxidized to produce a product that reacts with the probe to produce red color and fluorescence.
